Tran: Read this space for tips and help.
Purpose: Use the Action Planner now and in the future to keep track of school and community support services you use. For each support service, you can identify goals and times for involvement, keep notes on your progress, and plan next steps.
Tips:
1. Use this tool to list the supports you want from your school and community.
2. List the school and community supports, the person you know there, what you had to do to get the support and how well it worked.
3. Evaluate the supports you developed by writing the outcome, progress you made, and what you need to do next.
4. Take this tool to your IEP meeting and use it as an outline for conducting this part of the meeting.
