Activity Log/Plan card
Tran: Read this space for tips and help.
Purpose: Use the Activity Log/Plan to keep track of your recreational and volunteer activities and your enjoyment of them. You can use this form to go back and review your activities and make future plans.
Tips:
1. Use this tool to plan your leisure and recreational activities.
2. Keep a record of what you have done and how much each of them cost and give them a rating.
3. Keep a record of what you have done for others and where you did them and give them a rating.
4. On the summary page, list the things you like most and what you want to try next month.
5. Review your entries as you plan for your IEP meeting so you can discuss the things you like to do in your spare time.
