Purpose: Use the Finding Services tool to keep track of contacts for local agency services and the responses you receive.
Tips:
1. Use this tool when you need some help or service from an agency.
2. Begin by writing a description of the service you need, then list agency names and contact information with the services that match those you need.
3. Determine if you are eligible for services by calling or visiting the agency and then make notes about their services and if you want to set up the service.
4. Review your information when getting ready for you IEP meeting so you can plan for agencies that may be helpful to you in the future.
