Monitoring Card

Read this space for tips and help.

Purpose: Use the Monitoring Card to list the steps in a behavior goal and then monitor how well you do. You can take notes on how your plan worked and your outcome.

Tips:

1. When you break a goal down into steps, it is easier to get started and work your way to finish the goal.
2. Sometimes the hardest step is getting started, so be sure to decide something specific to do to get started.
3. Print out the card and keep it with you to track your success. Check off how you did on each step.
4. To evaluate your success, describe your success in the outcome box. Write notes to yourself on what to do different the next time you use this tool.
