Purpose: Use the Transition Planner to help you determine your goals for the future, think about resources you will need, and plan your school transition meeting.
Tips:
1. Use this tool when you are planning for your IEP transition meeting.
2. Enter the outcomes you hope to have and how you think the school can help you get them.
3. Enter community resources that you know will be able to help you now and once you have graduated.
4. List the people you want at your IEP meeting.
5. List the things you want to talk about during your IEP meeting.
